What Does it Look Like?

The safety band is a modified patient identity band. It is coloured green to represent go. Green bands are not currently used in the NHS for any other purpose.

 

 

 

 

How Does it Work?

Step 1

The Procedure box is filled out by the surgical team on the ward at the same time as the consent form. If the procedure has a side then this is written out in full.

The band is then initialed by the first doctor to confirm that he/she has checked the relevant notes, x-rays etc

Step 2

The patient is asked to confirm the procedure and then signs in the box provided for the patient

The band is then placed on the patient. The band can be attached to the wrist or ankle depending on the procedure and local preference. Hospital guidelines may state that patients can only leave the ward area without a band in a number of exceptional circumstances.

 

Step 3

When the patient reaches the operating theatre the operating surgeon and or anaesthetist must sign the band before the induction of anaesthesia.

Step 4

The band forms the focus of the STOP moment. This is led by the ODP cutting off the band. The ODP then presents a cross reference of the band, consent form and care sheet to the surgeon and scrub nurse. Only then does surgery begin.

Step 5

The band is attached to the care sheet to form an audit trial..
